Free light chains of monoclonal immunoglobulins are an important diagnostic marker of monoclonal gammopathy.

Monoclonal gammopathy or paraproteinemia is the collective name of a whole class of diseases in which there is a pathological secretion of abnormal, altered in chemical structure, molecular weight or immunological properties of immunoglobulins (gamma globulins) by one clone of plasma cells or B-lymphocytes. These immunoglobulins then disrupt the functions of certain organs and systems, such as the kidneys, which leads to the development of symptoms.

There are two types of immunoglobulin light chains: 1 kappa and 2 lambda. Serum concentrations of kappa and lambda light chains depend on the balance between plasma cell production and renal clearance.

In the case of plasma proliferative disorders, there is a secretion of excess monoclonal immunoglobulins or free light chains of immunoglobulins, which disrupts the normal balance between the kappa and lambda chains. The very kappa / lambda ratio that goes beyond is evidence of monoclonal gammopathy.
